The following is a list of appointments scheduled for you on Tuesday July 8^th.

Tuesday July 8^th

9:30am Mount Sinai Hospital – Pulmonary Function Test

4^th floor, Rm# 474. (Prior to this appointment, you will need to register for a Mount Sinai hospital card. This will be done at the Mount Sinai Hospital registration desk – main floor, and will take approximately 15minutes).

When you are finished at Mount Sinai Hospital, go to Princess Margaret Hospital. The remainder of the tests will be done at Princess Margaret Hospital.

11:15am Dental Assessment

Dental clinic, 2nd floor, Princess Margaret Hospital (Takes ~ 1 hour).

(Note: If any dental work needs to be done they will schedule additional appointments. If not covered by a dental plan the work will be charged).

Blood work

Clinic H south, 2nd floor. There will be blood work requisitions for you to pick up, at Clinic H south. Once you have these requisitions go to the Transfusion Centre to have your blood drawn.

Chest x-ray

Once the blood is drawn, go to the third floor for a chest x-ray.

Note: the blood work and chest x-ray may be done at any time prior to 1:30.

1:30 pm Clinic Appointment

Clinic H south, 2nd floor. You will be seen in clinic for review of blood work, history and physical, explanation of procedure, and consents. In addition, you will have a bone marrow aspirate and biopsy done. We will also review the dates and times of treatment. If any additional tests are required we will determine that during this appointment.
